This is a Collaborative Practice Agreement between a pharmacist and physician granting clinical privilege to the pharmacist in accordance with the Connecticut Pharmacy Practice Act. The purpose of this agreement is to optimize patient outcomes in the area of hypertension by integrating medication therapy management into the care of patients via a collaborative practice agreement between a pharmacist and a physician.
